This allows us to better prepare for your session and understand how we can best support you. You’ll find the form in the email titled:
'Welcome to Rebound - Confirmation of Your Appointment & Important Information.'

Please also view the following documentation prior to your appointment.
We can provide paper copies if you require, just let us know


How to find us

Rebound Health Brookvale

Our Address is 7/577 Pittwater Road, Brookvale. 

*You can enter our building from either Pittwater Road, or from Warringah Road, and there is parking available in the building. Once you have parked, please use the lift to go to Level 2. Exit the lift, turn left, and you will find us.


Rebound Health Warriewood

Our Address is 6/92a Mona Vale Road, Warriewood.

*Please note that you cannot access our location from Mona Vale Road - but rather from Daydream Street. 

Our designated parking spots are labelled '6' - so please park in these spots. 


What to bring

Images/reports/blood test results/any referral forms - anything you feel is relevant to us being able to best help you.

If you have a referral to see us, please bring that along as well.


Late Notice Policy

Please be aware that we have a 24 hour cancellation policy - if you can't make your appointment we request that you give us 24 hours notice so that we can help another client during that time, otherwise a fee may be charged (100% of the scheduled fee).  This is in line with the NDIS Pricing Arrangements and Price Limits. We understand that unexpected things can happen and you may not be able to make it.  If this is the case, please call us.
