Stroke Rehabilitation

When it comes to stroke rehabilitation, we generally begin working with our clients once they have been discharged from hospital.

We understand the role that neuroplasticity plays within stroke rehabilitation, that’s why we believe in a multi-disciplinary approach to care.

Our expert team will complete a comprehensive assessment and tailor a treatment plan to your goals and current abilities.

Treatment sessions commonly include:

  • Strengthening exercises to strengthen muscles paralysed or weak after your stroke

  • Arm therapy aimed at improving your arm function

  • Sessions can also address any pain or discomfort you are experiencing

  • Throughout your session you will receive 1-1 support from your physiotherapist, exercise physiologist or therapy assistant

  • Group exercise programs may also be recommended to focus on your fitness, balance or function - but we’ll discuss these options with you during your initial consultaiton with us.

Parkinson’s Disease

After years of working with people with Parkinsons Disease, we have developed our own tailored program for people - PD Power.

PD Power is an exercise rehab and therapy program based on the latest evidence of neuroplasticity. It involves physical and cognitive exercises that boost the brain’s ability to rewire and protect itself. This leads to a delay in onset of more severe symptoms, and an improvement in current symptoms.

Rebound Health is able to treat specific spinal disorders, provide rehabilitation plans post-spinal-surgery as well as treat general spinal symptoms and disorders from disability and aging.

We have clinical experience with scoliosis, lumbar spinal stenosis, canal stenosis, spina bifida, spinal arthritis, degenerative disc disease and most other common spinal disorders.

Short Term Restorative Care: Empowering Independence

At Rebound Health, we are dedicated to empowering our community members to live safely and independently in their own homes.

The Short Term Restorative Care (STRC) program, funded by the Australian Government, is designed to assist older adults in enhancing their functional abilities over an eight-week period. The goal is to enable individuals to maintain their independence for as long as possible.

Our skilled team of Physiotherapists, Occupational Therapists, Exercise Physiologists, and Dietitians collaborate with approved STRC providers to offer comprehensive therapy and support to clients enrolled in the program.

Initiating a Short Term Restorative Care Pan

Rebound Health collaborates with STRC providers to offer comprehensive support in Physiotherapy, Occupational Therapy, Exercise Physiology and Dietetics to clients.

The Aged Care Assessment Team (ACAT) grants approval for a Short Term Restorative Care package, assessing eligibility based on the following criteria:

- Age: Participants must be over the age of 65, or over 50 for Indigenous Australians and those of Torres Strait Islander descent.

- The potential to improve function, as determined by ACAT.

This program is part of the My Aged Care initiative, which provides support for older Australians.

For some individuals, a Short Term Restorative Care package serves as a preliminary step towards receiving further assistance through programs like the Commonwealth Home Support Program or the Home Care Package scheme.


What can STRC clients expect from Rebound Health?

STRC clients typically benefit from the expertise of our multidisciplinary allied health team, which includes Physiotherapists, Occupational Therapists, Exercise Physiologists, and Dietitians.

Most clients will engage in weekly Physiotherapy sessions, although some may require treatments twice a week.

Our Occupational Therapists are equipped to provide necessary equipment, often related to mobility and rehabilitation, such as:

  • Walking frames

  • Walking sticks

  • Exercise equipment

  • Wheelchairs

  • Electric recliner chairs

Occupational Therapists play a crucial role in assessing home modifications and assistive equipment needs. This process usually involves an initial assessment followed by consultations with builders or equipment suppliers, and may include equipment trials to identify suitable supports. Common home modifications for STRC clients include ramps, handrails, and bathroom modifications.

Our Exercise Physiologists provide support for clients in improving balance, reducing the risk of falls, increasing strength and improving independance.

Dietitians are integral to the STRC program, addressing the increased nutritional needs associated with enhanced physical activity and reviewing risks related to frailty or malnutrition in older age.

Clients already enrolled in a Short Term Restorative Care (STRC) Program have the opportunity to enhance their care by collaborating with Rebound Health. By communicating with their STRC coordinator, clients can request a connection with our team to arrange the necessary clinical treatments tailored to their specific needs.

To begin the process of determining eligibility for the STRC program, individuals or their caregivers can reach out to My Aged Care. This initial contact is a crucial step in accessing a wide range of services designed to support older Australians in maintaining their independence and quality of life.

Once the request is made, the Aged Care Assessment Team (ACAT) will conduct a comprehensive assessment. This evaluation is particularly important for those who are encountering difficulties in remaining safely at home due to various factors. Post-STRC Program Transition

Upon completing the 8-week Short Term Restorative Care (STRC) program, the team at Rebound health will collaborate with the client's Coordinator to develop a comprehensive exit strategy.

In many instances, our professionals continue to provide support to clients even after the STRC program concludes. This ongoing support is often necessary as clients may transition to a Home Care Package or become eligible for continued assistance through the Commonwealth Home Support Program.

For clients who do not have access to suitable funding options following the STRC program, Rebound Health offers services to private clients. This includes individuals with private health insurance, those eligible for DVA (Department of Veterans' Affairs) benefits, or those who can receive assistance through Medicare. Our goal is to ensure that all clients have the opportunity to continue receiving the care and support they need to maintain their independence and well-being.
